HOW TO ENTER: Complete the official entry form between [New Dates] (“Entry Period”) and submit it to Installation Made Easy (“Sponsor”). Limited to one entry per household. Any other attempted form of entry is prohibited. Entries that are lost, late, incomplete, or inaccurate will NOT be considered. You are NOT required to agree to have Sponsor’s representative contact you and/or attend/listen to a sales presentation in order to enter the Sweepstakes or win a prize. By entering, entrants agree to be bound by these Official Rules and by the decisions of the judges, which shall be final and binding.

ELIGIBILITY: This sweepstakes is open only to legal residents of the United States (the “Eligibility Area”) who are 18 years of age and older. Employees of BJs Wholesale Club and the Sponsor and their subsidiaries, affiliates, parent companies, or other entities involved with the sweepstakes (“Promotional Parties”) and members of their immediate families (biological or step), including parent, child, sibling, or spouse of any of the foregoing (regardless of where they live), or households of any of the above whether related or not, are NOT eligible to participate. Void outside the Eligibility Area and where prohibited by law.

DRAWING: Winners will be selected in a random drawing from all eligible entries on [New Date]. The drawing will be conducted by the Sponsor, whose decisions will be final and binding in all respects.  Odds of winning depend on the total number of eligible entries received during the Entry Period. Limit of one (1) prize per household.

PRIZE(S): Grand Prize [$$$$$$] Gift Cards will be sent via USPS certified mail to winners’ homes. Prizes CANNOT be transferred, assigned or redeemed for cash or substituted except at Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize of equivalent value if an advertised prize becomes or is unavailable for any reason.

CONDITIONS: The potential prize winners will be notified by e-mail approximately 10 days after the drawing and will be required to sign and return an Affidavit of Eligibility and Liability/Publicity Release (where permitted) within ten (10) days of the date printed on the prize notification, or the prize will be forfeited, and another winner chosen. All properly claimed prizes will be awarded. If the prize notification is returned as undeliverable, it will result in prize forfeiture. All federal, state, and local taxes on prizes and all other costs not specifically outlined in these rules are the sole responsibility of the winners.  By accepting the prize, the winner agrees to accept the prize in the manner set forth by the Sponsor and consents to the use of his/her name, address (city, state) photograph, likeness, and/or other indicia of identity for advertising and promotional purposes without limitation or additional compensation, except where prohibited by law. All winners acknowledge compliance with these Official Rules and agree to release the Promotional Parties from any and all liability and to hold the Promotional Parties harmless by winner and his/her heirs and executors for any injuries or damages of any kind sustained in connection with the use/misuse, acceptance, possession, or awarding of any prize.
